Who do I see?
We have introduced a new triage system for all patient GP appointment requests.
This means that instead of calling at 8am to book, all patients will now be asked to complete an online medical triage form. The form will be reviewed by a GP, who will decide the most appropriate next step for your care and ensure you are seen in the right place, at the right time, by the most appropriate professional.
The link to complete the online request form can be found here: https://accurx.nhs.uk/patient-initiated/d81025
If you have a medical issue, please select ‘I have a health problem’
If you have an admin request e.g. test results / fit note / Routine Care appointment such as vaccinations or request for information, please select ‘I have an admin or routing care request’
Please note – do not use admin request for medical issues. Unfortunately, they will not be dealt with and you will be asked to resubmit your request.

If you no longer need your appointment, please contact the practice on 01223 652221 to let us know as this will allow us to offer your slot to another patient.
This is very important as missed appointments waste valuable surgery time.
You can also cancel your appointment online by:
How do I submit Administrative Queries?
Accurx Online Patient Triage is an online consultation tool that allows you to submit any administrative request to Cherry Hinton & Brookfields Medical Centre.
What can I use it for?
Please consider an appointment with our ACP rather than a doctor. They can issue prescriptions and see children and adults for things such as Chest infections, ear, nose or throat complaints or other minor ailments .
They can also:
Assess diagnose and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
Assess diagnose and treat patients in surgery or own home who require acute medical attention and refer to appropriate agency.
Directly admit patients to secondary care hospital in acute medical need.
Refer patients to other medical specialities for assessment if deemed appropriate.
Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for imaging investigations (x-ray, ultrasound scan, MRIs).
As an alternative to an appointment we are able to offer telephone advice. Please advise the receptionist of your problem and they will arrange for an appropriate clinician to contact you.
All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. Please contact the reception team for further details or discuss with the clinician you are details.
Patients are requested to arrive in good time for their designated appointment. If you arrive late please let the reception team know.
Patients who arrive late could face a delay or will be invited to rebook, this is at the discretion of the clinician who your appointment was with.
Thank you for your co-operation
If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.
Disabled parking and access are available at both our surgeries.
We are part of the Cambridge GP network and therefore we are now offering evening and weekend appointments to deliver enhanced access (EA) service. EA are for any routine or urgent appointments that the patient wishes to have. It is not replacing the Out of Hours/NHS 111 service, but offers appointments at more convenient times. Patients can be seen in any of the six designated sites.
You can book your appointment through our receptionist who will give you a time, date and venue for your appointment. If you need to cancel or change your appointment please telephone 0330 0130 030.
Our receptionist will also at be able to answer any questions you have about the service.
