The practice catchment area extends from the Cambridge City and South East Route – Worts Causeway, Hills Road, Gonville Place, East Road, Newmarket Road, Airport Way followed by the link road taking in Teversham and Fulbourn and then on to Cherry Hinton via Fulbourn Road to Lime Kiln Hill.

In order to register with the practice we need each person who would like to register to complete a GMS1 form and a new patient health check questionnaire. We also require the immunisation history for children between 8 weeks and 16 years old. You can download these forms below or collect them from the surgery.
New Patient Health Check Questionnaire
We would recommend that you check your postcode through our postcode checker above to ensure your address is in our catchment area.
We will only ask for ID if you require Online Access, however we do encourage patients to download the NHS App.
Children wishing to register as patients with the surgery should also have a parent or legal guardian registered with us.
All patients that are registered at this practice have a named accountable GP. This GP is responsible for looking after you and coordinating your overall care.
Your named GP is assigned to you when you register at the practice but you are welcome to choose who your named GP is – please just let reception who you would like this to be next time you make contact with us.
If you are not sure who your named GP is, please ask any member of the team who will be happy to tell you. Alternatively if you receive any medication from us it will be noted on the B side of your prescription (the non-green side).
We will endeavour to ensure you see your named GP when you request to, however, if you require on-the day medical attention this may not always be possible. We do recommend that you become familiar with a couple of GPs so you can have continuity of care across the week (as not all GPs work every day) and when your named GP is on leave. All GPs have access to your medical record and will be able to take good care of you.
