For all routine appointments, please complete the online request form which you can find here: https://florey.accurx.com/p/D81025
We will aim to respond to these within 5-10 working days.
If your need is of medical urgency, please call the surgery at 8am on 01223 652221.
You will be asked a series of questions by our reception team – please be prepared to answer them as this helps us book you with the relevant clinician.
It is preferable for you to see your usual doctor for any on-going conditions and we recommend getting familiar with a couple of GPs if your preferred GP is part-time. However, if your problem is urgent and your doctor is not available, you will be given an appointment with the first available doctor that day.
Advanced Clinical Practice (ACP)
Please consider an appointment with our ACP rather than a doctor. They can issue prescriptions and see children and adults for things such as Chest infections, ear, nose or throat complaints or other minor ailments .
They can also:
Assess diagnose and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
Assess diagnose and treat patients in surgery or own home who require acute medical attention and refer to appropriate agency.
Directly admit patients to secondary care hospital in acute medical need.
Refer patients to other medical specialities for assessment if deemed appropriate.
Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for imaging investigations (x-ray, ultrasound scan, MRIs).
Telephone Advice
As an alternative to an appointment we are able to offer telephone advice. Please advise the receptionist of your problem and they will arrange for an appropriate clinician to contact you.
Chaperones
All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. Please contact the reception team for further details or discuss with the clinician you are details.
Late Arrivals
Patients are requested to arrive in good time for their designated appointment. If you arrive late please let the reception team know.
Patients who arrive late could face a delay or will be invited to rebook, this is at the discretion of the clinician who your appointment was with.
Thank you for your co-operation
