Practice Catchment Area

The practice catchment area extends from the Cambridge City and South East Route - Worts Causeway, Hills Road, Gonville Place, East Road, Newmarket Road, Airport Way followed by the link road taking in Teversham and Fulbourn and then on to Cherry Hinton via Fulbourn Road to Lime Kiln Hill.

Registering During Social distancing (Coronavirus)

In order to register you we require you to complete a GMS 1 form. This form is a legal document and without it we are unable to register you at the practice. This form also needs a ‘wet’ signature, as opposed to a typed or digital one.  Please print the GMS1 form, and send us a photograph of the completed form, with all details clear and legible.  Every patient needs their own form, regardless of their age.  If you do not have access to a printer, please call the surgery and we will post it out to you. Please be aware your registration will not be complete until we receive the completed GMS1 form and New Patient Questionnaire.

We also ask for some basic health information from you, as this helps us to provide suitable care to you.  Please complete the interactive New Patient Questionnaire form. 

We ask all our patients to provide evidence of their identify and address - this should be photographic ID, such as a Passort, Drivers Licence or UK Home Office Visa Card, and a utility bill, tenancy agreement or similar showing your name and address.  Please take a photo of yourself, your ID document and proof of address.

Once you have completed the GMS1 form, the new patient health check form and have photographed your ID please email them to us via capccg.cherryhintonmceds@nhs.net

Please be aware that you are emailing very confidential information about yourself via a potentially unsecure method; if you prefer to post your GMS form to us and have a video call to confirm your identity please contact reception (via telephone or email) and we will arrange this for you.  If you have any questions or do not have access to a smart phone / video call technology please contact the Reception Team who will be able to discuss  this with you further.
