New Patient Registration

Cherry_Hinton.jpgWe are pleased to accept new patients residing or moving into our practice area - see the 'practice catchment area' map below.

You will be registered with a named GP but you are free  to see whichever doctor you wish. However, we would advise you to become familiar with at least two GPs and see them for your on-going health needs.  For 'on the day' or medically urgent issues you may not be able to see your usual/named GP but we do have a wide range of clinical team members who can assist you. 

In order to register you need to complete a GMS1 and a new patient questionnaire.  These are available in hard copy at the practice or you can download them from the links above. 

We ask all registering patients to attend the practice in person with their forms, as well as photogrpahic ID and proof of address - a utility bill or rent agreement are ideal.

New Patient health Checks: Most new patients are offered a health check with a member of the healthcare team.  If you aged 40 - 74 years and do not have any pre-exisiting medical conditions you are entitled to have a "NHS health Check" - it should take about 20 minutes and is based on straightforward questions and measurements such as age, sex, family history, height, weight and blood pressure. There will also be a simple blood test to measure your cholesterol, blood sugar and your kidney function.

Medication: If you are on regular medication please be aware that we will not be able to issue this without you seeing a GP first.  Please ensure you make this appointment in plenty of time so you do not run out.

Practice Catchment Area

The practice catchment area extends from the Cambridge City and South East Route - Worts Causeway, Hills Road, Gonville Place, East Road, Newmarket Road, Airport Way followed by the link road taking in Teversham and Fulbourn and then on to Cherry Hinton via Fulbourn Road to Lime Kiln Hill.
